Geriatric Care Management

Job opportunities in geriatrics and long term care are expected to grow by more than 35% – ranking among the most rapidly growing employment areas, according to the Bureau of Labor Statistics. The home and community based care sector has grown to approximately four hundred million dollars per year and the provision of services in long term care is estimated to be as much as two billion dollars per year. To meet the demand for geriatric care professionals needed to manage this care, the University of Florida offers an online Graduate Certificate program.
The Geriatric Care Management Graduate Certificate program is designed to meet the needs of today’s working professionals. The program requires the successful completion of four graduate level courses with a grade of B or higher.
All courses are taught utilizing a web based e-learning system. The courses are organized in weekly modules which include audio PowerPoint presentations, video lectures, assigned reading and online discussion forums. The material is designed to be completed in self-paced/flexible format. Upon completion of the Geriatric Care Management Graduate Certificate students will be able to:
- Apply general principles and knowledge necessary to integrate theory into the practice of care coordination and management
- Demonstrate the ability to assess and identify client strengths, needs, concern, preferences and establish goals and a comprehensive plan of care
- Coordinate services and interventions by referring, educating, negotiating, and mediating with client formal and informal providers in order to meet goals of a care plan
- Perform ongoing monitoring and reassessment of client status and satisfaction with service in order to evaluate progress toward goal achievement
- Exercise sound professional judgment based on ethical standards and expectations of the field including advocacy for client autonomy, professional accountability, and peer review
- Demonstrate the ability to conduct and/or evaluate appropriate research on effective case management techniques for provision of services to aging clients and their families.
Upon completion of this program graduates will have the expanded knowledge required for either developing their own private practice Professional Geriatric Care Management Business or advancing their career in management of Long Term Care Facilities, Home Health Care or Public Service Agencies. The program is also designed to meet the educational requirements for those seeking to become a Certified Geriatric Care Manager through the International Commission on Health Care Certification.

GEY 6646: Issues & Concepts in Gerontology (Community Resources for Geriatric Care)
This introductory course provides an overview of the multidisciplinary field of gerontology, aging services, and community resources for geriatric care management. Material will be covered that informs students about the basic biology, psychology, and sociology of the aging process. Patterns of service use and public and private resources to meet the individual needs of geriatric clients are examined. Principles of case management and the role and function of the private geriatric care manager are presented.
GEY 6220: Overview of Geriatric Care Management
This course presents the basic elements and procedures for conducting a comprehensive geriatric assessment and care plan report. The content of the course covers the fundamentals of a psychosocial and functional assessment. Current issues in dementia and depression are discussed. Emphasis is placed on conducting the client interview, gathering all necessary information, and incorporating that information into a cohesive written care plan.
GEY 6306: Communication in the Aging Network (Family Systems & Care Planning)
This course examines the role of communication in successful care plan implementation and coordination. Students will explore the impact of family systems on the provision of care for individuals and aging families. Topics include caregiver assessment, care monitoring, helping mid-life children work together to care for aging parents, preparing for and facilitating family meetings, forgiveness and effective use of technology.
GEY 5935: Ethical, Legal and Business Aspects of Geriatric Care Management
This course explores the ethical and legal issues encountered in the practice of geriatric care management including informed consent, decision making capacity, advanced directives and the regulation and financing of long term care. The course will also address issues associated with starting a business in Geriatric Care Management including entrepreneurial risk, marketing, fee for service billing, and human resources. Emphasis is placed on creating a business plan.
GEY 6936: Clinical Experience
Minimum Requirements for Acceptance
- Applicants must have a baccalaureate degree, preferably in a health-related or social service discipline, e.g. Nursing, Social Work, Occupational Therapy, Physical Therapy, Gerontology, Sociology, Health Education, etc.
- A GPA of 3.0 or better
Applying to the Program
Applications are accepted on a year round basis. Newly admitted students will begin the program in the Fall or Spring semester of every year. This program is designed for working professionals and in order to apply to the program you will need to submit the following:
- A Nondegree Registration Request (PDF)
- A short (one page) essay describing why you want to be in the program and how this training will advance your professional development career aspirations
- A copy of your current resume (including two years of work experience in a health related field)
- A copy of all of your academic transcripts

e-Learning in Sakai requires one of the following web browsers:

Will a slow typist get behind in class?
The only time a slow typist may have a little trouble is in a chat session, but even this disadvantage is not a big deal. Otherwise, slow typists should do fine, even though they may of course require a little more time to finish some assignments and/or projects.

Faculty & Staff
Jamie Pomeranz, Ph.D., CRC, CLCP is an Assistant Research Professor within the Department of Behavioral Science and Community Health. Dr. Pomeranz has obtained three degrees from the University of Florida which include: Master’s of Exercise and Sports Science, Master’s of Health Science in Rehabilitation Counseling and Doctor of Philosophy in Rehabilitation Science. He has worked with people with disabilities in multiple capacities for over 19 years. His clinical background includes working with individuals with spinal cord injury, traumatic brain injury, and developmental disabilities. Dr. Pomeranz has been on faculty at the University of Florida since 2004 and his topics of research include: tobacco cessation for individuals with disabilities, life care planning for returning veterans with catastrophic injuries, and life care planning implications of locomotor training for individuals with spinal cord injury.
Amanda Glynn, MHS, CRC, joined the department of BSCH in 2010 as the Continuing Education Program Coordinator for the Canadian Life Care Planning, Domestic Life Care Planning, Medicare Set-Aside and Geriatric Care Management Programs offered. She received her Bachelor’s degree in Health Science with an emphasis in Rehabilitation Services from the University of Florida. In addition, she received a minor in Education. She received her Master’s in Health Science with an emphasis in Rehabilitation Counseling from the University of Florida. Amanda is currently a Certified Rehabilitation Counselor, and Certified Geriatric Care Manager.
Program Faculty & Course Facilitators
Keith Meneskie, MHS, CMC, CDP RMHCI, CGCM, has 18 years of geriatric care management experience in sub-acute, rehabilitation facilities and professional private practice. He has extensive knowledge of Advanced Directives, Medicare, Medicaid (state of Florida), Veterans Administration criteria and guidelines for eligibility. Mr. Meneskie currently navigates across all types of elder living environments including private homes, assisted living facilities, adult family care facilities, subsidized housing, and nursing homes. Mr. Meneskie assists with and recommends alternative placement options when the current environment is unsuitable for the overall well being of the elder client.
Mr. Meneskie is an active member of the Florida Geriatric Care Managers Association and the National Association of Professional Geriatric Care Managers, National Council of Certified Dementia Practitioners, and The National Academy of Certified Care Managers, State of Florida Registered Mental Health Counselor Intern.
Mr. Meneskie received a Bachelor of Science in Psychology degree from Kennesaw State University and the Master of Health Science Degree in Rehabilitation Counseling from the University of Florida. He is a Certified Care Manager, Certified Dementia Practitioner, Registered Mental Health Counselor Intern, and Certified Geriatric Care Manager.
Laura J. Perry, Ph.D., CRC, CLCP, and a Registered Mental Health Counselor Intern. She maintains a small practice in individual and family services, providing mental health counseling and geriatric care management in the greater Gainesville area. She specializes in helping individuals and families navigate issues related to aging, disability, and long-term care planning. Dr. Perry received her Ph.D. from the University of Florida. She taught rehabilitation counseling for a number of years in the Department of Behavioral Science and Community Health and has previously taught the communication course in the Geriatric Care Management Certificate Program. Dr. Perry is a member of the Association for Adult Development and Aging and the American Rehabilitation Counseling Association, divisions of the American Counseling Association; she is also a member of the Life Care Planning and Case Management divisions of the International Association of Rehabilitation Professionals.
