Geriatric Care Management Admissions

Admission Requirements


  • Must have a baccalaureate degree, preferably in a health-related or social service discipline, e.g. Nursing, Social Work, Occupational Therapy, Physical Therapy, Gerontology, Sociology, Health Education, etc.
  • Must have a GPA of 3.0 or above

How to Apply


STEP #1 Complete the UF Application.

  1. Please scroll down to certificate and click on, “I am NOT a currently enrolled UF student”
  2. You will need to select Geriatric Care Management Program
STEP #2 You may upload your unofficial transcripts, but you will have to send in your official sealed transcripts to the Office of Admissions.
  1. By e-mail: The University of Florida is able to accept official transcripts if the school uses one of the following systems to send the transcripts through email: Parchment or Faster. If so, you can have them email it to the following: certificates@admissions.ufl.edu
  2. By mail: Transcripts can be mailed to the Office of Admissions at UF:

STEP #3 Submit a short (one page) essay describing why you want to be in the program and how this training will advance your professional development career aspirations AND a copy of your current resume (including two years of work experience in a health related field)
